ASSURANT HEALTH (formerly Fortis Health)
is a Fortune 500
company traded on the New York Stock Exchange under the symbol AIZ. Assurant
has over $26 Billion in Assets and $8 Billion in annual revenue. Assurant
Health's multiple underwriting companies provide health insurance for more
than one million people nationwide. The companies' primary products include
individual medical, small group, short-term and student health insurance
products, as well as non-insurance products and consumer-choice products
such as Health Savings Accounts. With almost 3,000 employees, Assurant
Health is headquartered in Milwaukee, Wisconsin and has operations offices
in Minnesota, Idaho and Florida, as well as sales offices across the U.S.
Assurant Health
is the brand name for products underwritten and issued by one or more of
Assurant Health's underwriting companies Time Insurance Company, John Alden
Life Insurance Company and Union Security Insurance Company. ASSURANT
Health Plans provide high and low deductible plans with $35 unlimited
doctor co pays without first paying the calendar year deductible. With
100%, 80/20, and 50/50 co-insurance plans, R/X cards are optional with a
$15 co pay for generic prescription drugs with no yearly deductible first,
and $25 co pays for brand name drugs after a once a year $500 deductible has
been satisfied. Well care is included immediately with up to $750 per person
allowance yearly on the Max Plan for laboratory tests, tuberculosis test and
colonoscopy (subject to deductible). A co pay is all that is required for
office visit charges related to well care visits & well care
immunizations. $500 per person well care allowance on the Core Med Plan
available after 6 months of policy ownership. Mammograms, Pap Smears & PSA
tests (prostate) would be covered by the plan but subject to
deductible.
Surgical
treatment of tonsils/adenoids will not be covered for a period of 3 months
after the policy has been issued. Surgical treatment of bunions,
hemorrhoids, inguinal hernia (except strangulated or incarcerated) and
varicose veins will not be covered for a period of 6 months after the policy
has been issued. Sterilization will not be covered for a period of 12 months
after the policy has been issued. These waiting periods are waived when
these plans are replacing other similar in-force health insurance coverage.
It should be noted that Time Insurance company has a very long history of
superior claims payment performance. However, since Assurant Health went
public as in independent company (separate from Fortis A in Belgium and
Fortis B in the Netherlands) there have been several recent news reports
