Personal Health Plans provide high and low deductible plans with unlimited "first dollar" outpatient doctor office visit co pays. Each visit will cost only a $40 co pay. A "first dollar" benefit is defined as "a benefit you receive without paying your calendar year health  plan deductible first e.g. outpatient doctor office visit co pays, outpatient prescription drug co pays, outpatient  preventative care". They also provide very competitively priced HSA qualified HDHP's with 100% and 80% "co- insurance" options to choose from.

With 100%, 80/20, 70/30 and 50/50 co-insurance plans discount drug cards are included at no additional cost. Also available for an additional premium on all plans is an outpatient "first dollar" co pay prescription drug card. With this option added, Generic prescription drugs will be covered with no deductible required. Brand name drugs will be covered for a $50 co pay after a separate calendar year $500 or $1,000 outpatient brand name prescription drug deductible is satisfied. 

Personal Health Plans now include the new mandates under the PPACA (Patient Protection & Affordable Care Act) that went in to effect on September 23rd, 2010. They are as follows:

  • No lifetime limits on overall plan benefits, and no lifetime limit on the dollar value of specific "essential benefits."
  • Annual limits on essential benefits, only until 1/1/14.
  • Rescission only for fraud or intentional misrepresentation of material facts. (This was law before the PPACA).
  • Coverage of preventive health services and immunizations without cost sharing. Including the following

    • Abdominal Aortic Aneurysm one-time screening for men of specified ages who have ever smoked
    • Alcohol Misuse screening and counseling
    • Aspirin use for men and women of certain ages
    • Blood Pressure screening for all adults
    • Cholesterol screening for adults of certain ages or at higher risk
    • Colorectal Cancer screening for adults over 50
    • Depression screening for adults
    • Type 2 Diabetes screening for adults with high blood pressure
    • Diet counseling for adults at higher risk for chronic disease
    • HIV screening for all adults at higher risk
    • Immunization vaccines for adults--doses, recommended ages, and recommended populations vary:
      • Hepatitis A
      • Hepatitis B
      • Herpes Zoster
      • Human Papillomavirus
      • Influenza
      • Measles, Mumps, Rubella
      • Meningococcal
      • Pneumococcal
      • Tetanus, Diphtheria, Pertussis
      • Varicella
    • Obesity screening and counseling for all adults
    • Sexually Transmitted Infection (STI) prevention counseling for adults at higher risk
    • Tobacco Use screening for all adults and cessation interventions for tobacco users
    • Syphilis screening for all adults at higher risk

    Covered Preventive Services for Women, Including Pregnant Women:

    • Anemia screening on a routine basis for pregnant women
    • Bacteriuria urinary tract or other infection screening for pregnant women
    • BRCA counseling about genetic testing for women at higher risk
    • Breast Cancer Mammography screenings every 1 to 2 years for women over 40
    • Breast Cancer Chemoprevention counseling for women at higher risk
    • Breast Feeding interventions to support and promote breast feeding
    • Cervical Cancer screening for sexually active women
    • Chlamydia Infection screening for younger women and other women at higher risk
    • Folic Acid supplements for women who may become pregnant
    • Gonorrhea screening for all women at higher risk
    • Hepatitis B screening for pregnant women at their first prenatal visit
    • Osteoporosis screening for women over age 60 depending on risk factors
    • Rh Incompatibility screening for all pregnant women and follow-up testing for women at higher risk

  • Prohibition of pre-existing exclusions for individuals under 19.
  • Compliance with specific internal claims appeals processes and external review requirements.
