Advantage P.P.O. Major Medical Health Insurance
policies are underwritten
by Guarantee Trust Life Insurance Company, Glenview, Illinois licensed in 49
states. Founded in 1936. The "Advantage" Major Medical Health Insurance P.P.O.
policy provides $1,000 to $2,500 deductible
plans with $25 unlimited doctor co pays without first paying
the hospital deductible. 80/20, 75/25, and 50/50 co-insurance plans are
available. R/X cards are included with $15 co pays for generic
and
$30 co pays for formulary brand name prescription drugs, with a NO
deductible first. Routine Physical Exams are covered after 90 days of
policy ownership on a first dollar basis up to $150 per examination. The
exam includes coverage for lab tests (blood/urine) associated with the same
routine physical exam. No pay is required. Benefits will be paid at the
same co-insurance level you chose for your health plan. Mammograms Pap Smears, Prostate Cancer Screening
tests as well as Colorectal cancer screenings are also covered from day one
with no deductible required first with the "Enhanced Routine Preventative
Care" selected. 4th Quarter Deductible Carry over is also included
with the plan as well as an Accident Medical Expense plan equal to whatever

The acronym "H.S.A" is being tossed around quite a bit nowadays
especially since the tax advantages of owning an HSA and a
corresponding qualified HDHP (High Deductible Health Plan) have been
significantly increased under the Bush administration. HSA stands for
Health Savings Account. Health Savings Accounts are a unique way to

visit (up to 7 visits per insured per year) is covered up to $100. There are
no co pays or deductibles that have to be met with the "Health
Max Plus" plan. All benefits provided are covered on a "first dollar"

receiving workers' compensation or on Medicare you are automatically
approved! All plans have a 12 month waiting period for pre existing
conditions, however they are HIPAA compliant so credit will be given towards
satisfying that waiting period for those coming from another guarantee issue

high and low deductible plans with $35 unlimited doctor co pays
without first paying the hospital deductible. With 80/20 and 70/30
